Seeking Registered Nurse for Ohel’s integrated Behavioral Health Clinic to provide mental health, substance abuse treatment and physical health monitoring. Successful candidate will:

• Be a lead member of the Integrative Behavioral Health team, enrolling and monitoring clients into the Certified Community Behavioral Health Clinic Services
• Provide primary care screening and monitoring of key health indicators and health risks (e.g., BMI, blood pressure, tobacco use).
• Document client care, and collaborate with other members of the patient care team.
• Manage and coordinate the health care needs of clients to improve access to care and promote physical and emotional well being
• Provide high quality, skilled, whole-health services within a rehabilitative and therapeutic model of care
• Actively engage with clients, developing positive relationships that support the client in meeting their goals
• Act as a collaborative team member leading and addressing comprehensive health promotion
• Facilitate preventative and restorative health and well-being education in individual, group and community settings
• Act as a liaison between the client and external care providers for whole health needs
• Provide information and/or referrals to resources in the community for increased support and stabilization
• Offer coaching on medication management
• Document observable behaviors and symptoms
• Provide 1:1 support and intervention for people served as needed.
• Participate in meetings and communication with other providers as needed.

Current NYS RN license and a preferred 2 years of post-graduate experience. Experience in mental health a plus. Excellent salary and benefits with a flexible schedule. This part time position is located in Far Rockaway.
