Case Manager- Apartment Treatment

Seeking high energy and responsible individual to work collaboratively with individuals who have a Mental Illness in Ohel’s Apartment Treatment Program. Our goal is to support their wellness, recovery and independence through case management services. Responsibilities include: advocacy, regular apartment visits, medication and medical monitoring, contacting client’s mental health and medical health teams on a monthly basis. Must be able to appropriately document services provided, fire drill evacuations and apartment inspections. Position requires a Bachelor’s degree in the Human Services field with strong background working with individuals who have a Mental Illness, Masters preferred. This full time position is based in Far Rockaway. Valid driver’s license and own car preferred.

Salary: Bachelors Level $55,000; Masters Level $65,000
