<div> <p></p> <p>Ohel is seeking a Care Coordinator to manage the care of children enrolled in Ohel's Health Home program. The Care Coordinator will assess the child’s physical, mental health and social services needs and will be responsible for developing an integrated plan of care, working collaboratively with medical, behavioral, educational and social service providers. The Care Coordinator will also provide care coordination and health promotion, transitional care and follow up, individual and family support, referrals to community and social support services as well as the use of health information technology to link services. Position requires a Bachelor’s Degree and 2 years experience providing case management or working one on one with children with behavioral health needs. This full time position is based in Brooklyn and requires monthly home visits.</p> <p>Salary: Bachelors Level $50,000</p> <p> Masters Level $55,000</p> </div>
