Ohel is looking for Case Manager to work with Older Adults diagnosed with severe and persistent mental illness who live in the Brooklyn and Staten Island Community. With the goal of older adults aging in place, the candidate will provide case management services, referrals to appropriate community connections and resources, as well as supportive counseling.

Responsibilities:

• Perform in-home assessments and reassessments of homebound older adults and develop comprehensive care plans
• Provide extensive and ongoing case management services including information, case assistance, entitlement counseling, referrals, supportive contact, advocacy and crisis intervention
• Keep detailed and comprehensive records of all contact with clients, including case files and unit entries in the database

Required Qualifications:

• Bachelors Degree in Health-related field required, Masters preferred
• Strong interpersonal, clinical, organizational skills as well as experience with benefits, entitlements and resource applications
• Experience in working with the older adult population

The ideal candidate is a self-starter who can work independently as well as within a team and has with excellent follow up skills. This full time position is based in Staten Island and Brooklyn, offers a hybrid schedule, with home visits as needed. Position includes a competitive benefits package, including health insurance, graduate school scholarship program, as well as generous paid time off.

Salary: Bachelors $50,000- $55,000

Masters $60,000 - $65,000
