Treatment Coordinator - Behavioral Health

Treatment Coordinator: The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing behavioral health needs. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.

Responsibilities:
- Performs ongoing coordination of services to children and adults in accordance with the treatment plan.
- Responds to initial crisis calls and identifies needs & necessary resources accordingly.
- Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services while demonstrating a positive, strengths-based attitude of teamwork and shared leadership.
- Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.
Qualifications:
- Bachelor's Degree in a human services field preferred; a combination of experience and education may be considered
- Relevant experience working in a behavioral-health setting
- Level 1 Fingerprint Clearance Card
- Verification of education, employment, references
- MVR matching JFCS driving guidelines
- AZ Driver's License and Valid Auto Insurance
