Overview

Westchester Jewish Community Services (WJCS), a nonprofit founded in 1943, has grown to be the largest provider of licensed outpatient community mental health services and one of the largest human service agencies in Westchester County. We provide state-of-the-art programs and services and compassionate care to more than 20,000 people of all ages and backgrounds each year.

We are seeking a full-time Care Manager to join our innovative High Fidelity Wraparound (HFW) team. HFW is an evidence-based process that supports youth with severe emotional and behavioral needs. HFW provides team-based planning, that is comprehensive, well-coordinated, holistic and family/youth-driven, with the intent to meet the complex needs of youth with serious mental health needs. The goal is to bring all parties to the planning table, to support a plan that is responsive to the family’s identified needs.

The salary for this full-time position is $55,000 - $60,000, and provides extensive training.

Responsibilities

As a valued member of our team, your responsibilities will include, but are not limited to:

  • Provide care coordination services to youth/adolescents, up to age twenty-one.
  • Follow, to fidelity, the HFW process and procedures set forth by OMH.
  • Provide crisis intervention and prevention services as necessary.
  • Coordinate the care for program participants and work closely with a variety of providers and family members to ensure the youth's success and positive mental wellness.
  • Provide safety planning to prevent psychiatric or medical hospitalization.
  • Maintain records to according to agency policies and procedures and to regulatory agencies directives.

Requirements:

We are seeking candidates who possess the following qualifications:

  • Bachelor’s degree in Social Work or related field with a minimum of 2 years of relevant experience working with children/adolescents or a Master’s Degree with one-year experience.
  • Ability to work with youth/adolescents/young adults from 5 to 21 years old who are impacted by serious emotional needs and/or chronic medical needs.
  • Meeting facilitation skills, strong collaboration skills.
  • Ability to engage and work with parents/caregivers.
  • A driver’s license and vehicle are required.
  • Bilingual English/Spanish preferred.
