Older Adult, Caregiver Support Case Manager

Job Summary:
The Older Adult/Caregiver Support Case Manager assists a client in the coordination and delivery of multiple social and casework services. The case manager is supervised by the Coordinator of Older Adult Services and Coordinator of Caregiver Services. The Older Adult/Caregiver Support Case Manager responds to community/client requests for services and information in an efficient and effective manner. The position requires strong clinical, assessment and crisis intervention skills as many cases are multi-layer requiring an integration of counselling, advocacy, and case management approaches.
Duties and Responsibilities:
- To assist in the coordination of the delivery of multiple services including, concrete, advocacy, volunteer, information and referral, and psychoeducational support on behalf of clients within communities.
- Provide case management and caregiver support services through in person contact or by phone. In person contact may be in public spaces (such as township offices or libraries) or when called for, within a client’s home.
- Develop strong collaborative relationships with clients to facilitate the collaborative case management process of reviewing financial details to assess needs, develop a plan towards self-sufficiency, providing counselling, referrals, exploration of ambivalence and linkage, while fostering client empowerment. Collect all appropriate data and help determine what role financial assistance may play in client’s service delivery plan.
- Provide support, social and psychoeducational groups to members of the community based on identified needs.
- Facilitate support groups for caregivers.
- Responsible for the JFS caregiver hotline.
- Provide ongoing support and resource linkage under the phone-based care consultation program through the Benjamin Rose Institute.
- Document case management clients and caregiver support group clients into EHR and complete progress notes to document all contacts with clients. Share up to date data of case management services based on Agency/grant specific needs and requests.
- Complete weekly statistics, enter client and caregiver information into the CCIS database through the Benjamin Rose Institute program. Provide statistical reporting information as identified through the CCIS database and provide information as needed for agency internal assessments and outside funding sources.
- Participate in Person Centered Trauma Informed training and apply to delivery of services.
- Provide a continuity of appropriate care/service, with goals of enhancement of autonomy and independence by improvement of maintenance of functioning. Assist in identifying unmet service needs, and to seek out those resources-internal and external to JFS, which would close the gap between what is needed and available. Research, identify, and access resources, or other activities, related to obtaining or providing services and supports for own clients as well as clients agencywide. Also responsible for developing relationships with resources and being up to date on services, accessibility, and requirements. Resources should be kept in a current and organized manner so that other staff can access as needed.
- Participation in JFS meetings
- Partake in regular supervision with the Coordinator of Older Adult Services and Coordinator of Caregiver Services.
Qualifications:
- Master’s degree in Psychology, Social Work, or Marital and Family Therapy is preferred.
- Bachelor’s degree with relevant case management experience will be considered.
- Work experience within a social service milieu, delivering services of some kind to client population.
- Licensure in the state of NJ as an LAC or LSW.
- Ability to communicate clearly verbally and in written form, organize and manage workload, and keep records.
- Familiarity with and sensitivity to Jewish identity (including its religious, ethical, cultural, and social values).
- Mature development of oneself as a professional, with demonstrated knowledge of engagement relationship, and rapport skills.
- Knowledge of JFS mission, and identification with JFS purpose and mission.
