SOMA Case Manager (Part-Time)

Job Summary:
The Case Manager is responsible for assisting clients in the coordination, delivery, and management of multiple social and casework services to support their well-being and overall quality of life. This role involves assessing client needs and facilitating access to appropriate resources and support systems. The Case Manager works under the supervision of the Coordinator of Older Adult Services and collaborates with various community organizations, and service providers to ensure comprehensive care and assistance for clients.
Duties and Responsibilities:
- Coordinate the delivery of a comprehensive range of services, including advocacy, volunteer support, information and referral, and psychoeducational assistance, to meet the needs of clients in the South Orange and Maplewood communities.
- Collaborate with professional staff within the South Orange and Maplewood communities to provide social work services in alignment with the guidelines and mission of the agency.
- Deliver services through in-person meetings or phone consultations. In-person meetings may take place in public locations, such as township offices or libraries, or, when necessary, within a client’s home.
- Facilitate support groups, social gatherings, and psychoeducational programs for community members based on identified needs.
- Maintain accurate and up-to-date client records by entering case details into the EMR and documenting all client interactions through progress notes.
- Administer the modified Geriatric Depression Scale (GDS) for all new clients and conduct reassessments at designated intervals to monitor changes in mental well-being.
- Compile and submit weekly statistical reports to track service delivery and client engagement.
- Participate in Person-Centered, Trauma-Informed training and integrate these principles into service delivery.
- Ensure continuity of care by facilitating access to appropriate services, with the goal of enhancing autonomy, independence, and overall well-being.
- Identify gaps in available services and advocate for additional internal and external resources to better meet client needs.
- Participate in ongoing supervision with the Coordinator of Older Adult Services to ensure professional growth and service effectiveness.
Qualifications:
- A bachelor's degree in psychology, social work, marital and family therapy, or a related field is required, master's degree preferred.
- Relevant work experience in a social services setting, providing direct services to clients.
- Active licensure in the state of New Jersey as a Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) preferred.
- Understanding of Jewish values and the ability to incorporate them into service delivery within an agency context, when appropriate.
