The Marion & Aaron Gural Jewish Community Center, a non-profit, non-sectarian community agency with four locations in the Five Towns is looking for a social worker to work with older adults. This is an on-site, full-time role Job Responsibilities:- Conduct intakes for our older adult programs and other JCC programs and services.- Perform outreach to synagogues, local organizations, physician offices, health care agencies, government agencies and community members.- Provide external referrals as appropriate.- Perform crisis intervention, short-term counseling and connect to concrete services.- Develop and maintain a resource guide for community referrals- Coordinate all aspects of assigned older adult program activities including transportation if needed- Ensure implementation of the agencys policies and practice- Provide psycho-social client intakes and Person-Centered Care Plans- Communication with clients, caretakers and families- Provide an environment of dignity and respect for all clients
