Social Worker -Community Support Services Specialist, Older Adults

The Marion & Aaron Gural Jewish Community Center, a non-profit, non-sectarian community agency with four locations in the Five Towns is looking for a social worker to work with older adults.

This is an on-site, full-time role

Job Responsibilities:

• Conduct intakes for our older adult programs and other JCC programs and services.
• Perform outreach to synagogues, local organizations, physician offices, health care agencies, government agencies and community members.
• Provide external referrals as appropriate.
• Perform crisis intervention, short-term counseling and connect to concrete services.
• Develop and maintain a resource guide for community referrals
• Coordinate all aspects of assigned older adult program activities including transportation if needed
• Ensure implementation of the agency's policies and practice
• Provide psycho-social client intakes and Person-Centered Care Plans
• Communication with clients, caretakers and families
• Provide an environment of dignity and respect for all clients

Job Requirements:

• MSW or LMSW preferred with two to three years of experience working with Older Adults
• Must be very familiar with Jewish practice and observance
• Ability to work with diverse teams and clients.
• Program development and outreach
• Strong time-management and organizational skills.
• Ability to multi task and problem solve
• Excellent verbal and written communication skills
• Grant writing experience a plus

Pay: $50,000.00 - $55,000.00 per year
