Schedule: Monday, Tuesday, Friday, and every other weekend.
MTF 8:30-4:30 pm, and Saturday, Sunday 8:30 am 2 pm

The Opportunity:
We need you to join our team as an Admissions Coordinator
The Admissions Coordinator is responsible for managing the Health Center admissions process and for establishing and maintaining an open, effective communication between the facility and referral sources, physicians, and third-party payers.
Key responsibilities include:
- Assist the Manager of Admissions in obtaining referrals from hospital social workers, physicians, or family members, keeping wait lists current.
- Assist in providing tours of the facility and pertinent information regarding admissions.
- Assist the Manager of Admissions with admission interviews with the resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information, medical information, attending physician information, and other pertinent and required documentation prior to admission.
- Assist the Manager of Admissions in reviewing all admission-related paperwork with the resident and/or responsible party, including Medicare Part A coverage and non-coverage procedures. Maintain all non-medical record-related paperwork in an admissions file for each resident.
- Assist in maintaining ongoing census activity in the EMR, such as admissions, discharges, deaths, room transfers, changes in payment methods, etc.
- Assist in maintaining Life Care resident statistics, permanent assignments, temporary assignments, and notify the resident and/or the responsible party of permanent and temporary status in the Health Center.
- Assist in maintaining an ongoing daily/monthly census of residents in the Health Center, noting admissions, discharges, deaths, transfers, changes in payment methods, and temporary and permanent assignments.
- Maintain complete confidentiality regarding admission information and resident records.
- Assist in coordinating admissions/transfers/discharges with other departments.
- Assist the Manager of Admissions in ensuring that the resident, family, and responsible person are fully informed of the resident’s personal and property rights.
QUALIFICATIONS:
Good working knowledge of Medicare Part A and certification procedures. Experience in a long-term care setting is desired. Must possess excellent written, oral, and interpersonal skills, as well as the ability to juggle multiple priorities.
PHYSICAL REQUIREMENTS/WORKING CONDITIONS:
Normally works in a well-lighted, comfortable office setting. Must be able to work efficiently with frequent interruptions. Must be able to move freely throughout the facility and be able to lift (unassisted) 50 lbs. May be subjected to offensive odors and exposure to chronically ill and frail elderly. Must be free from communicable diseases.
