Position Summary: Plans, organizes and coordinates the operations of the program by performing the following functions directly or through delegation to appropriate subordinate staff members.

Principal Responsibilities:

  • Develops, implements and maintains departmental projects, policies and procedures, objectives, quality assurance and environmental standards, in conjunction with Programs Director.
  • Reviews completed projects, makes recommendations, and reports on program activities to Programs Director.
  • Assigns and coordinates work projects.
  • Supervises social work staff.
  • Supervises students.
  • Supervises volunteers
  • Provides case management services to clients, including assessment; information and referral; applications for entitlements and benefits; developing case plans; arranging for, and monitoring, provision of services; advocacy on behalf of clients; counseling.
  • Organizes and follows up on outreach to community.
  • Represents the organization at community meetings, committees, and inter-agency councils.
  • Performs client intake duties by phone or walk-in. Takes information from client or collateral (e.g., family members, physician, etc.) to determine eligibility and need.
  • Continues to maintain and enhance knowledge about entitlement programs, benefits and resources for our specific client population.
  • Maintains case notes and records, and required documentation, according to departmental standards and/or regulatory requirements.
  • Prepares statistical and other reports, as required.
  • Manages program budget.
  • Manages completion of reports to funding sources.
  • Manages NY Times Neediest Program for Selfhelp.
  • Participate in staff meetings and in-service trainings.
  • Performs other related duties as assigned or requested.
  • Adheres to organization’s standards by completing annual mandatory trainings in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Adheres to the organization’s policy in regards to absenteeism and appearance and health and safety standards.

Salary Ranges: $75,000 - $77,400 per year commensurate with experience.

Job Competencies & Minimum Qualifications:

  • Master's degree in Social Work (MSW) required, SIFI certified preferred. At least two years of case management services to elders living in the community
  • Supervises the social work staff, students and volunteers; interviewing and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems
  • Computer literate
  • Excellent verbal and written skills
  • Excellent customer service skills
  • Able to multi-task and work independently with a great attention to detail

Working Conditions/Physical Demand:

  • Business office environment with phone and computer use.
  • Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
  • Occasional exposure to toxic or caustic chemicals, outside weather conditions, risk of exposure to pathogens, and unsanitary conditions in client’s homes.
