Job Overview
Ruth and Norman Rales Jewish Family Service is currently hiring a full time Holocaust Service Geriatric Case Manager. This position is responsible for assessing individuals and couples capabilities to remain safely at home or assist them in developing alternate living arrangements. Assists in gaining access to needed medical, social, educational, psychological and other essential services. Assess clients’ strengths and limitations and develop a plan of care. Coordinate services, linkage to community resources and monitor overall service delivery. Effectively communicate with all service providers and family members.

Required Experience and Education

• Masters in Social Work
• LCSW preferred
• Minimum 2-5 years experience in geriatric case management
• Effective verbal and written communication skills
• Demonstrates ability to resolve problems and provide assistance and teamwork where appropriate.
• Strong organizational skills as reflected in effective scheduling, planning and meeting deadlines.
• Requires flexibility with scheduling on an as needed basis.
