Case Manager for A.B.M. Program

Organization: Mosholu Montefiore Community Center (MMCC)

Job Location: Hamilton Heights, Manhattan (10032)

Status: Full-Time, In-Person

Reports to: Director of Support Services Department

Schedule: Monday–Friday, 8-hour shift (8:00 AM–5:00 PM or 9:00 AM–6:00 PM) with occasional alternating Saturdays as needed.

Annual Salary: $47,000 – $50,000, commensurate with education and experience

Position Summary: MMCC is seeking a Full-Time Case Manager for the A.B.M. Support Services Program located in Hamilton Heights, Manhattan. The Support Services Department has extensive experience advocating for, supporting, and stabilizing individuals and families experiencing housing and financial instability.

The Case Manager will work both independently and collaboratively to assist vulnerable community members in accessing essential social services. This role involves assessing client needs, developing service plans, coordinating resources, and supporting clients in achieving housing stability and improved well-being.

Key Responsibilities:

● Conduct comprehensive client intake assessments in the office, clients’ homes, or other service settings.

● Collect demographic documentation and verify sources of income and eligibility for benefits.

● Identify unmet needs and evaluate existing services clients may already be receiving.

● Assist clients with applications, recertifications, and follow-up for public benefits and housing programs.

● Maintain detailed case notes, progress reports, and event logs in accordance with agency standards.

● Monitor and evaluate client progress and service outcomes.

● Coordinate services and collaborate with internal staff, community partners, and service providers.

● Provide referrals to housing, employment, financial, and social service resources.

● Maintain organized documentation and ensure timely reporting and recordkeeping.

● Participate in team meetings, trainings, and community events as required.

● Perform additional duties as assigned by the Program Director.

Qualifications:

● Bachelor’s degree required, preferably in Psychology, Counseling, Social Work, Human Services, Special Education, or a related field.

● Experience assisting clients with Human Resources Administration (HRA) benefits, including applications, recertifications, and follow-up.

● Familiarity with Section 8 and NYCHA housing processes, including applications and recertifications.

● Experience with grant applications and rental arrears resolution, including coordination with property management and funding sources.

● Experience supporting housing stabilization and placement services.

● Strong proficiency in Microsoft Office Suite, including Excel, Publisher, and PowerPoint.

● Excellent time management and organizational skills, with the ability to manage multiple cases simultaneously.

● Strong written and verbal communication skills.

● High attention to detail with strong problem-solving and critical thinking abilities.

● Knowledge of community resources and social service systems strongly preferred.

● Must have a reliable means of transportation and be willing to travel to client homes, residential facilities, hospitals, schools, and community-based programs.

● Ability to climb up to 20 flights of stairs and lift up to 15 pounds when necessary.
