Case Manager for NDA Program

Status: Full-Time, In-person

Reports to: Multisite Supervisor

Schedule: Monday – Friday, 8:00 am – 5:00 pm

Annual Salary: $49,000 – $52,000, commensurate with education and experience

Position Summary:
MMCC is seeking a dedicated and compassionate Case Manager for the NDA Healthy Families program. This role is responsible for supporting individuals and families by connecting them to essential NYC benefits and social services. The ideal candidate is experienced in case management, comfortable working independently, and committed to advocating for underserved communities.

Responsibilities:

● Assist clients in applying for and obtaining public benefits, including SNAP, Medicaid, Cash Assistance, One Shot Deals, SCRIE, Social Security, and NYC ID.

● Conduct intake assessments to determine client eligibility and needs.

● Manage a caseload, including crisis intervention and ongoing support.

● Maintain accurate, timely case notes and documentation in DYCD databases and internal systems.

● Ensure all client files are organized and audit-ready in compliance with DYCD standards.

● Assess client needs, including financial, medical, and social factors, to develop comprehensive service plans.

● Coordinate referrals to housing, education, employment, mental health, and other community resources.

● Advocate on behalf of clients and collaborate with external service providers.

● Monitor client progress and adjust service plans as needed.

● Maintain strict confidentiality in accordance with HIPAA and agency policies.

● Ensure compliance with NDA program requirements and performance targets.

Qualifications:

● Bachelor’s degree in Social Work, Human Services, or a related field required.

● Knowledge of NYC benefits systems, including SNAP, Medicaid, and Cash Assistance.

● Experience in case management, crisis intervention, or working with vulnerable populations.

● Strong organizational, communication, and documentation skills.

● Ability to manage multiple cases in a fast-paced environment.

● Bilingual (English/Spanish) preferred.
