LOCATION: Residential Program - Highland Park, IL
REPORTS TO: Director of Adult Programs
JOB STATUS: Full-time / Exempt
POSITION SUMMARY:
The primary purpose of the QIDP is to provide case management services to the assigned CILA individuals. The QIDP is responsible for developing and overseeing the implementation of the Personal Plan for individuals served. They are directly responsible for facilitating the achievement of personal outcomes and for overseeing the delivery of services across settings in order to enhance the well-being, quality of life, opportunities and achievement of personal outcomes for individuals served on their caseload.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Developing, coordinating, and overseeing the delivery of services across settings to enhance the well-being, quality of life, opportunities, and achievement of individuals served on their caseload.

Develop and monitor the implementation of the Personal Plan to address Personal Outcome Measures, including chairing and organizing related plan meetings.

Maintain all clinical documentation as required by program standards and licensing entities.

Oversee preparation for and participate in program surveys by accrediting bodies such as DHS, BALC, BQM, etc.

Ensures safety, health, and wellbeing of residents by responding to emergencies and crisis situations.

Collaborate with the Nurse to ensure documentation and required medical/dental/personal care appointments comply.

Setting up visits and observations
-
Collaborate with Operations Associate and House Managers on DSP training
-
Train new staff on resident personal plans, implementation strategies, behavior support plans, Therap responsibilities, etc.

Complete DSP training reimbursement requests
-
Develop and maintain active case files to ensure that all appropriate forms are completed and filed in a timely manner
-
Track monthly activities and progress of goals and report with monthly summaries

Oversee Therap to ensure all staff are writing t-logs, tracking goals, and filling out applicable incident reports

Track resident attendance and compile attendance reports for billing including tracking 1:1 hours for individuals with 53R funding

Complete all required DHS paperwork such as 53R packets and service agreements
-
Communicate and comply with ISC agency as needed
-
Sit on various Committees to ensure compliance with DHS
-
Fill in as a GADOL DSP when needed and as time permits
-
Collaborate with Behavior Support Company to ensure compliance with Behavior Support Plans

In times of need, may be asked to complete related duties other than those indicated above
Qualifications
QUALIFICATIONS:
- Bachelor’s Degree
- 2 years of experience working with adults with intellectual/developmental disabilities
- Valid Driver’s License
- CPR and First Aid Certificate
- DSP Certification
- QIDP Certification
