JSSA is looking for a full-time Nurse Practitioner to support our Montgomery County and Northern Virginia end of life patients and their families.
Responsibilities:
- Clinical assignments are performed with minimal supervision.
- Conduct comprehensive physical exams, evaluate eligibility for hospice, and monitor for disease progression and symptom changes.
- Develop, implement, and update personalized care plans, focusing on comfort and aligning with patient/family goals.
- Prescribe medications and therapies, manage side effects, and also provide non-pharmacological interventions for symptom relief.
- Work closely with hospice physicians, nurses, social workers, chaplains, and bereavement counselors to ensure holistic care.
- Maintain thorough records of assessments, evaluations, consultations, and patient progress. Document patient care in accordance with Agency policy and Medicare/Medicaid and other third-party requirements.
- Perform home visits, facility visits, and sometimes inpatient evaluations.
- Perform face to face visits in person and utilize telehealth on occasion (if law allows)
- Assist with referral and admission process to review patient eligibility
- Signs death certificates and MOLST/POLST forms as delegated by the hospice physicians.
- Participate in the on call schedule which necessities a shared and equitable rotation to cover holidays, weeknights and weekends
- Have sufficient knowledge of the Medicare Hospice Benefit and other third-party insurances including the ability to interpret these to the patient and family
- Performs other appropriate duties as required
Qualifications:
- Master's or Doctoral degree in Nursing from an accredited nurse practitioner program.
- Advanced Certified Hospice and Palliative Nurse (ACHPN) highly preferred.
- Minimum of 2 years of nurse practitioner experience, preferably in hospice, palliative care, geriatrics, or internal medicine.
- License/Certification Active and unrestricted Maryland and Virginia NP License and NPI number required. Must be willing and able to make in person visits in both Maryland and Northern Virginia.
- State and Federal controlled substance registration and DEA number required for prescribing.
- Experience providing care in home and nursing homes/assisted living facilities.
- Valid driver's license and reliable transportation to travel across Montgomery County and Northern Virginia as needed for visits.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills with patients, families, and interdisciplinary teams.
- Proficiency with Homecare Homebase or other hospice EMR systems preferred.
- Knowledgeable in the Hospice Conditions of Participation and Hospice Eligibility as outlined in the Local Coverage Determination (LCD) Guidelines
