JSSA's Hospice team has an immediate opening for a PRN Nurse Practitioner (NP) to deliver high-quality, compassionate care to terminally ill patients and their families in Montgomery County on Fridays, Saturdays and Sundays.
Responsibilities:
- Performs and documents in-person Hospice Face-to-Face visits to assist the Hospice Physicians in the certification and recertification process. If telehealth visits become permissible, face-to-face visits may also be conducted via telehealth.
- Performs skilled APN visits to assist with symptom management as needed.
- Signs death certificates and MOLST forms as delegated by the hospice physicians.
- Updating the patient’s medical chart in the electronic medical record in a timely fashion
Qualifications:
- Master's or Doctoral degree in Nursing from an accredited nurse practitioner program.
- Advanced Certified Hospice and Palliative Nurse (ACHPN) highly preferred.
- Minimum of 2 years of nurse practitioner experience, preferably in hospice, palliative care, geriatrics, or internal medicine.
- License/Certification Active and unrestricted Maryland NP License and NPI number required.
- State and Federal controlled substance registration and DEA number required for prescribing.
- Experience providing care in home and nursing homes/assisted living facilities.
- Valid driver's license and reliable transportation to travel across Montgomery County as needed for visits.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills with patients, families, and interdisciplinary teams.
- Proficiency with Homecare Homebase or other hospice EMR systems preferred
- Knowledgeable in the Hospice Conditions of Participation and Hospice Eligibility as outlined in the Local Coverage Determination (LCD) Guidelines
Rates of pay: $250 per FaceToFace Visit/$150 per Telehealth Visit
