JSSA Hospice is looking for a nurse Care Coordinator to assist patients and their families during the end-of-life process.

Responsibilities:

• Support primary nurses and doctors with handling clinical and scheduling/administrative hospice calls regarding patient care
• Oversee the scheduling and productivity of the hospice aides (CNAs/CMTs)
• Provide administrative support in preparing for and during IDG meetings
• Assist with any office administrative tasks as needed
• Attend all required meetings
• Available to work onsite Monday through Friday from 7:30am-4pm
• Able to make filed visits to perform annual and as-needed hospice aide supervisory visits

Qualifications:

• Active LPN or RN license to practice in Maryland
• 3-5 years’ experience performing related work
• Demonstrated organizational and time management skills
• The ability to multitask and work in a fast-paced environment
• Demonstrated flexibility, adaptability, and outstanding customer service skills
• Proficient with MS Office 365(Teams, Word, Excel), Electronic Medical Records and other databases
• Familiarity and comfort with hospice medical terminology
• Ability to manage confidential information with sensitivity
• Outstanding phone and in person customer service skills
