Hospice Triage LPN
JSSA Hospice is looking for a Triage nurse to assist patients and their families during the end-of-life process.
Responsibilities:
- Support primary nurses and doctors with handling clinical and scheduling/administrative hospice calls regarding patient care
- Oversee the scheduling and productivity of the hospice aides (CNAs/CMTs)
- Provide administrative support in preparing for and during IDG meetings
- Assist with any office administrative tasks as needed
- Attend all required meetings
- Available to work onsite Monday through Friday from 7:30am-4pm
- Able to make filed visits to perform annual and as-needed hospice aide supervisory visits
Qualifications:
- Active LPN license to practice in Maryland
- 3-5 years’ experience performing related work
- Demonstrated organizational and time management skills
- The ability to multitask and work in a fast-paced environment
- Demonstrated flexibility, adaptability, and outstanding customer service skills
- Proficient with MS Office 365(Teams, Word, Excel), Electronic Medical Records and other databases
- Familiarity and comfort with hospice medical terminology
- Ability to manage confidential information with sensitivity
- Outstanding phone and in person customer service skills
