Transitions Coordinating Nurse - LPN
JSSA is looking for a full-time LPN to join our homecare/hospice team and provide support to our Transitions program.
Responsibilities:
- Provide for the safe discharge of patients from hospice into community care settings.
- Provide check-ins (in-person and telehealth) to recent hospice discharges and those with advanced illness that are not considered eligible for hospice.
- Work closely with the Hospice Intake department, Admission nurses, Hospice Social Workers, Liaisons, and other key hospice IDG team members to ensure quality and timely care of patients who become eligible for hospice services.
- Work closely with other JSSA departments to ensure that clients receive comprehensive and coordinated care that may be available within the Agency.
Qualifications:
- Active LPN license to practice in MD required.
- Valid driver’s license and reliable vehicle required.
- Able to make in-person client visits as needed.
- Able to drive throughout Montgomery County.
- Minimum 1 year of Hospice and/or homecare experience in a clinical role.
- Clear understanding of complex illnesses.
- Highly effective verbal communication skills including courtesy, resourcefulness with the ability to answer questions and explain hospice/departmental policies and procedures.
- Proficient in the use of electronic medical records, and Microsoft Office 365 applications.
- Comfortable with navigating the home care, assisted living and nursing home environments.
- Must be able to work Monday through Friday from 8:30am-5pm.
