Full job description

Job Summary: The Medical Doctor, PRN (M.D.), will serve a broad array of Agency clientele. The MD will provide clinical services to help determine the best course of treatment for patients at risk for developing dementia or diagnosed with dementia. The provider works within a team of care managers and nurses who deliver wraparound services to individuals to ensure coordination of care and other needed support services. The M.D. has the authority to prescribe medications, order tests, and maintain plans.

Duties and Responsibilities:


  • Clinical Services include but is not limited to; seeing (i) JFSA patients either at the Clinic or during home visits as requested by JFSA and agreed to by the provider; and to (ii) devoting time to responding to calls and maintaining appropriate records.

  • Per clinic service, the provider’s responsibilities during each patient-care session include:

§ Providing geriatric and dementia/cognitive assessments,

§ Coordinating medical care with internal members of the health care team and external medical specialists; and

§ Maintaining appropriate communication regarding medical-related issues with families and/or guardians.

  • Non-clinical service activities include but is not limited to; attending via telephone conference team and/or staff meetings on a schedule mutually agreed by the Parties; leading educational sessions on topics of aging and dementia, as requested by JFSA and agreed to in advance by the provider; and maintaining appropriate documentation and records when additional time is required beyond that which is provided for individual patient-care sessions.
  • Working within a team that delivers integrated services to individuals to ensure coordination of care and other needed support services.
  • Providing directions to other team members will delegate tasks according to capabilities and scope of responsibilities.
  • Documenting and operating timely and effectively within the Agency’s Electronic Health Record.
  • Collaborating with interdisciplinary team members, including physicians, psychiatrists, psychologists, care navigators and nursing staff, to help develop care plans.
  • Monitoring the effectiveness of interventions.
  • Providing education and consultation to the Agency care management staff to ensure coordination of care and improved health outcomes.
  • Conducting follow-up visits including, but not limited to, medication refills, determining the need for follow-up care, and making appropriate referrals, according to policy.
  • Developing and implementing treatment plans that lead to improved health outcomes.
  • Prescribing appropriate medications for physical and mental health issues demonstrating knowledge of drug action, interaction, and reactions.
  • Functioning independently to perform age-appropriate history and physicals for complex acute, critical, and chronically ill clients.

  • Reports to the appropriate staff, in a timely fashion, any unsafe activities, hazards, or safety violations that may cause injury to oneself, other employees, patients, and family members.

  • Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices. Also contributes to the control of hazards in specified area of responsibility.

  • Performs other duties or special projects as required or as assigned.

Minimum Qualifications


Educational/Training Requirements:

  • Medical Degree from accredited school of medicine
  • Training as an Adult, Acute Care, Family or Geriatric Doctor is highly desirable.


Experience:

  • Experience working within a healthcare system, preferably in Cleveland, Ohio

Licenses/Certifications:

  • Current Licensed Medical Doctor in Ohio


Knowledge, Skills and Abilities:

  • Demonstrated mastery of clinical concepts and principles and ability to identify and use appropriate concepts and principles from related sciences; e.g. behavioral sciences and biological sciences.
  • Excellent analytical and decision-making skills are required.
  • Fluent Russian oral and written language skills a plus.
  • Strong written and verbal communication skills.
  • Demonstrated competency and the ability to perform advanced assessment and diagnosis.
  • Demonstrated ability to function independently and in collaboration with medical and associated staff.
  • Strong computer skills, including Word, and Excel .
