JFSA of Cleveland has an immediate opening for an Activities of Daily Living Support Specialist ( S-CPST) to provide services to adults diagnosed with severe and persistent mental illness (SPMI).
This role is ideal for those looking to educate clients on activities of daily living, such as shopping, budgeting, healthy habits, and traveling. You play an essential role in monitoring clients’ mental status and general day-to-day community functioning by supporting the Agency's Mental Health and Therapeutic Behavioral Specialists.
Duties and Responsibilities:
- Reliably provides supportive services to adult SPMI.
- Provides psychoeducational functions regarding SPMI, Med/Som (modification and symptom management).
- Instructs clients on ADL’s and teaches other community living skills.
- Transports clients to services and community resources.
- Reports on changes in client functioning to other care staff and supervisors.
- Provides / or facilitates access to crisis stabilization services as needed.
- Fosters opportunities to form relationships with others for support, sense of participation in community life and self-worth.
- Assists in monitoring mental status and general functioning. Monitors the effects of medications and facilitates compliance.
- Accountable to supervisors in maintaining client documentation in accordance with requirements.
- Maintains thorough knowledge of the strengths, goals and personal plans of consumers assigned to their care unit.
- Collaborates with peers to meet the needs of consumers and facilitates illness management and recovery process.
- Completes billing documentation in a timely fashion and in accordance with SCPST service standards.
- Meets productivity targets.
- Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices. Also contributes to the control of hazards in specified area of responsibility.
- Drives his/her personal auto between assignments, and transport clients as required.
- Performs other duties or special projects as required or as assigned.
Minimum Qualifications
Educational/Training Requirements:
- High School graduate or GED.
- Two yrs. of college in social sciences preferred.
Experience:
- 1-2 yrs experience in community support services.
Licenses/Certifications:
- Maintain both a valid Ohio Drivers License, and a driving record that meets JFSA’s insurance company standards (e.g. less than 2 incidents over a 3-year period)
- Maintain personal auto liability insurance at the level that the Agency and the insurance carrier requires.
- Must have a reliable personal automobile available daily.
Knowledge, Skills and Abilities:
- Knowledge of the community where services are provided.
- Prior work with SMI & DD populations.
- Ability to organize and use time effectively.
- Ability to be patient and tolerant.
- Good communication & writing skills.
