Full job description

Community Care Manager

The Community Care Manager is responsible for the supervision, oversight, and performance management of Community Care Coordinators while also providing direct care support as needed to ensure continuity, safety, and quality of services. This role ensures services are delivered in accordance with Individual Service Plans (ISPs), regulatory standards, agency policies, and the cultural and religious expectations of Orthodox Jewish households. The Community Care Manager serves as the primary on-site leader, balancing supervisory responsibilities with hands-on support during coverage gaps, transitions, emergencies, and complex situations.

This is a full-time, salaried role that requires working flexible hours, including evenings, weekends, holidays, and on-call coverage as required.


Staff Supervision & Performance Management

  • Provide consistent, active supervision of Community Care Coordinators across assigned sites
  • Ensure Coordinators understand and correctly implement ISPs, service plans, and daily routines
  • Conduct regular check-ins, performance reviews, and skills coaching
  • Address performance concerns promptly, including corrective action when necessary
  • Ensure staff compliance with agency policies, safety standards, and professional conduct
  • Support hiring, onboarding, and disciplinary processes in collaboration with management and HR

Service Oversight & Quality Assurance

  • Monitor service delivery to ensure ISP goals are met with fidelity
  • Review and approve documentation for accuracy, timeliness, and compliance
  • Identify service gaps, risks, or trends and escalate appropriately
  • Ensure programming is delivered consistently and at a high level of quality
  • Confirm medication administration and health-related supports are carried out per protocol

Cultural & Religious Compliance Oversight

  • Enforce kosher kitchen standards and household food handling requirements
  • Ensure staff compliance with Shabbat, Jewish holiday observance, and restricted activities
  • Provide guidance and corrective instruction to staff regarding religious routines
  • Maintain privacy and confidentiality related to religious practices in the home
  • Serve as the point of escalation for cultural or religious concerns

Operations, Safety & Compliance

  • Oversee day-to-day site operations and staffing coverage
  • Ensure timely reporting and follow-up of all UIRs and MUIs
  • Lead response to crises and emergencies, ensuring appropriate communication and documentation
  • Prepare for and participate in compliance, licensure, and quality reviews
  • Enforce safety policies and hazard prevention practices

Communication & Collaboration

  • Serve as the primary liaison between Community Care Coordinators and management
  • Communicate effectively with families regarding day-to-day service concerns as appropriate
  • Collaborate with nursing, service coordination, and interdisciplinary team members
  • Support transitions, move-ins, and service changes

Administrative Responsibilities

  • Monitor site-level operational needs and staffing patterns
  • Review service hours and documentation for billing accuracy
  • Maintain required supervisory presence in addition to administrative responsibilities

Educational/Training Requirements:

  • High School Diploma or equivalent required
  • College coursework or Bachelor’s degree preferred


Experience:

  • Minimum of 3–5 years’ experience supporting individuals with intellectual and/or developmental disabilities
  • Demonstrated supervisory or leadership experience required


Licenses/Certifications:

  • Working knowledge of Orthodox Jewish practices impacting daily living and knowledge of kosher laws and Jewish holiday observance
  • Ensures all essential training requirements are completed, i.e., annual training, CPR/First Aid, certified medication passing, etc.
  • Maintain both a valid Ohio Driver’s License, and a driving record that meets JFSA’s insurance company standards (i.e. no more than 2 incidents over a 3-year period) in addition to all regulatory bodies
  • Maintain personal auto liability insurance at the level that the Agency and the insurance carrier require.
  • Must have a reliable personal automobile available daily.

  • Excellent written and verbal communication skills.

  • Ability to maintain confidentiality and professional boundaries.
  • Proficiency with electronic documentation systems and Microsoft Office.
  • Excellent ability to relate to families of clients.
  • Computer skills, including ability to proficiently use Microsoft Office Suite.
  • High level of motivation and ability to take initiative.
  • Exceptional organizational skills and time management skills.

Physical Demands:

  • The physical demands are typical of similar jobs in comparable organizations.
  • Must be able to operate a motor vehicle and manage significant travel.
  • Must have stamina to work long hours.


Work Environment:

  • The work environment is representative of and typical of similar jobs in comparable organizations.
  • Must have a personal vehicle available daily for client transport.
  • Must have stamina to work long hours and be able to lift up to 50 pounds
