Full job description

Immediate opening for a full-time Licensed Practical Nurse (or Medical Assistant) to work one-on-one with individuals with complex medical needs in a home care environment. The health care provider will work under the direction of a Registered Nurse and in accordance with the individual plan of care. The provider will be closely monitoring health conditions, including oversight of medication regimen, client safety, and providing timely updates on any decline in well-being. Client homes may contain pets, and hours of work may vary, including overnights, weekends, and holidays.

Job Requirements:

  • Licensed Practical Nurse
  • High School Diploma/GED
  • Valid LPN license in Ohio and current CPR/First Aid Certification
  • Maintain both a valid Ohio Driver’s License and a driving record that meets JFSA’s insurance company standards.
  • Maintain personal auto liability insurance at the level that the Agency and the insurance carrier require.
