The Intake Social Worker at the Jona Goldich Multipurpose Senior Center provides responsive and client-centered support to individuals seeking social services, community-based assistance, and information and referrals. This role is responsible for managing both phone and walk-in inquiries, conducting intake assessments, and providing short-term case management support as needed. The position also supports outreach activities, including community workshops, information sessions, and engagement events designed to connect older adults and adults with disabilities to essential services and resources.
QUALIFICATIONS:
- Minimum of a Bachelor’s degree in Social Work required or related human services field; Master’s degree preferred
- Prior experience in client engagement, intake services, customer service, or social service delivery required
- Strong research skills to identify and connect clients with appropriate community resources
- Excellent verbal and written communication skills
- Bilingual in English/Russian preferred or willingness to utilize language line interpretation services
- Demonstrated ability to work with diverse, multicultural populations in a culturally sensitive manner
- Strong interpersonal skills and ability to provide non-judgmental client support
- Comfortable presenting in group settings and supporting outreach events and workshops
- Ability to work collaboratively within a multidisciplinary team
- Strong computer proficiency, including MS Office and ability to learn electronic health record (EHR) systems
- Ability to maintain professional boundaries with clients and staff
- Valid California Driver’s License and proof of current auto insurance required
- Must successfully pass criminal background check, including Live Scan fingerprinting, prior to employment
RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
- Provide intake, engagement, and time-sensitive support to incoming phone calls and walk-in clients at a multipurpose senior center serving older adults and adults with disabilities
- Conduct intake assessments, including bio-psycho-social and environmental evaluations
- Provide information, referrals, and service linkage based on client needs
- Triage clients for social work case management and coordinate appropriate follow-up services
- Assist clients with applications for housing, public benefits, and other supportive services
- Conduct home visits as required to assess client safety and complete intake documentation
- Support outreach efforts, including workshops, presentations, and community events
- Collaborate with case management, nutrition, and activities staff to enhance client engagement
- Research and identify community resources to support case management staff and client needs
- Assist with completion of grant applications and benefit-related documentation
- Maintain timely and accurate documentation in the electronic health record (EHR) system, ensuring all notes are completed daily
- Participate in multidisciplinary team meetings and weekly supervision sessions
- Manage multiple priorities effectively while maintaining quality service delivery
- Perform other related duties as assigned
