The Central Access Department Social Worker II will provide crisis intervention, case management, navigation, and linkage to internal or other community-based programs and services. The Social Worker II will provide outreach to agency partners and community-based agencies. The position is both field and office-based and services will be provided both in person and through telehealth.
Qualifications:
- Master’s degree in social work or related field; license eligible required
- Current unrestricted CA Driver’s License and proof of auto insurance
- Prior experience with client/ customer service engagement, and provision of case management required
- Field-based service delivery and in-home visits experience preferred
- Knowledge of and ability to research relevant community resources required
- Ability to target and navigate needed resource/government benefits for multiple client populations preferred
- Must have engaging and strong communication skills both verbal and written and be able to actively utilize telehealth platforms.
- Strong computer literacy skills in all MS Office applications required as well as ability to quickly learn database systems for electronic records and data collection.
- Culturally sensitive and able to work with a multicultural client/customer population; ability to work in a non-judgmental manner and foster positive outcomes; able to work as part of a team and maintain professional boundaries.
- Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment
Responsibilities/Essential Functions:
- Respond to intake calls and assess for client service delivery presenting issues
- Conduct in-depth intake and targeted case management assessment
- Develop targeted case management plan/goals in collaboration with client
- Provide crisis intervention as needed and follow up
- Provide preventive care services and regular follow up
- Assist clients in applying for basic grants and other eligible government benefits
- Assist with coordination of family or other informal support for clients
- Provide system navigation, linkage to community resources
- Navigate warm handoff to community behavioral health providers
- Navigate warm handoff to community social service providers
- Assist clients with coordination of transportation for follow up doctor’s or other appointments as needed
- Provide timely documentation in keeping with funding requirements and electronic health records
- Participate in multi-disciplinary team meetings as scheduled
- Participate in weekly individual supervisory meetings
- Supervised hours towards licensure available but not guaranteed
- Other related duties as assigned
