Geriatric Care Manager

JFS is looking for a motivated Care Manager to provide high-quality services to adults and families. The Care Manager is responsible for providing comprehensive case management services to clients.

The ideal candidate will have experience working with seniors and adults with disabilities. The ability to effectively communicate and serve as a liaison between the client and the families served, and various community agencies is of the utmost importance.

Duties and Responsibilities: The Care Manager’s responsibilities include but are not limited to the following:

Client Services:

  • Addresses intake calls with potential clients and provides accurate and detailed information
  • Completing comprehensive assessments, services plans and quarterly reviews.
  • Provides comprehensive care management services to clients
  • Ongoing collaboration and engagement with community resources available to support and meet the needs of older adults and adults with disabilities, as well as the needs of families and caregivers.
  • Engagement with the current standards and criteria set by JFS for serving in the role as Guardianship, Conservatorship, Advanced Medical Directives, and Power of Attorney, as well as a comprehensive understanding of the legal criteria for eligibility.
  • Knowledge of entitlements, eligibility guidelines and the application process.
  • Maintain a balanced caseload and meet billable expectations.
  • Works in partnership with the care management team and interns to meet clients’ needs.
  • Participate in on-call rotation

Compliance:

  • Professional, timely and accurate completion of paperwork as it relates to direct client care.
  • Ongoing review and updates of client’s care plan

Program Needs:

  • Networking and relationship building with community partners to facilitate Care Management services for clients served.
  • Providing support and consultation to Interns.
  • Attendance and active participation at all agency and departmental meetings.
  • Other assignments as determined by Chief Program Officer and Care Services Director.

The ideal candidate will have experience working with seniors and adults with disabilities. The ability to effectively communicate and serve as a liaison between the client, and the family served, and various community agencies is of the utmost importance. The ideal candidate will have a degree in Social Work, Human Services, or be a Registered Nurse.

Salary: $43,000-$50,000

Benefits: Company paid Dental, Life/ADD, and LTD. Accrue PTO and sick time, floater days and holiday pay. After 1 year, eligible to participate in 401(k) with company match. Access to subsidized medical insurance, STD, Vision and additional voluntary life.
