Please note this position must reside in San Diego County to visit clients' homes
Reports To: Director of Aging Connections
Pay Range: $55-$65/ year

In addition to standard pay, compensation for this position includes:

Position Overview:
JFS is looking for a compassionate, mission-driven individual to join the Aging Care Services team as the Occupational Therapist for Home Modification Programs. In this role, you have the opportunity to use your passion and skills and experience to make a meaningful difference in your community and help change lives across San Diego. The Occupational Therapist plays an integral role on the team and supports the entire agency by enhancing the safety, accessibility, and functionality of our clients' homes, allowing them to live independently and with dignity. The Occupational Therapist will be responsible for assessing and recommending home modification and fall prevention strategies for older adults by working closely with clients, caregivers, and other team members to identify risks. This position reports directly to the Director of Aging Connections and works closely with the Fix-it team and the rest of the Aging Care Services staff.
Responsibilities:
- Conduct comprehensive home safety assessments for older adults
- Conduct comprehensive home assessments to identify environmental barriers and safety hazards
- Develop individualized home modification plans based on client needs, preferences, and best practices in occupational therapy
- Collaborate with clients, caregivers, and case managers to ensure effective communication and implementation of modification plans
- Provide education and training to clients and caregivers on the use of adaptive equipment and modified home environments
- Document assessments, plans, and progress notes accurately and in a timely manner
- Stay current with advancements in occupational therapy and home modification techniques and technologies
- Participate in team meetings and contribute to the continuous improvement of the Home Modification Program
Skills/Experience/Abilities That Are a Must-Have:
- Master's or Doctoral degree in Occupational Therapy from an accredited program
- Current state licensure as an Occupational Therapist
- Minimum of 2 years of experience in occupational therapy, with a focus on home modifications or geriatrics preferred
- Must have a valid CA driver's license, insurance, a reliable personal vehicle, and a willingness to travel off-site
- Strong knowledge of home safety, fall prevention, and adaptive equipment
- Ability to work independently and manage a flexible schedule
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Skills/Abilities We’d Like You to Have:
- 2 years of experience working with older adults, preferably in home health or community settings
- Certification in Home Modifications (e.g., Executive Certificate in Home Modification, CAPS)
Physical Requirements:
- Ability to travel to clients' homes and conduct physical assessments
- Capability to perform physical tasks related to assessing home environments and recommending modifications
- Occasionally lift and/or move up to 20 pounds
