Pay Range: $24-$25/hour

Position Overview:
JFS is looking for a compassionate, mission-driven individual to join the Adolescent Family Life Program team as a case manager. The Adolescent Family Life Program (AFLP) is a case management program for expectant and parenting youth aged 21 and younger to: increase social and emotional support and build resiliency, improve pregnancy planning and spacing, increase education attainment and employability, and increase access to needed services. Case managers will be utilizing the evidence-informed Positive Youth Development (PYD) Model to improve the life course trajectory of expectant and parenting youth through a resiliency-based, youth-led program. Case managers play an integral role on the team and support the entire agency by providing supportive case management services to
expectant and parenting youth. This position reports directly to the AFLP Program Manager and works closely with the AFLP team and the rest of the Parenting & Youth Services department
Responsibilities:
- Provide supportive case management services to 25 adolescents per month in a community, school, in-home setting, or telehealth
- Regular travel to client homes or client identified community locations to provide case management services, transport support items, or provide transportation support around San Diego County
- Develop individual plans with each client to meet the health, psychosocial, educational, vocational, and financial needs of pregnant and parenting adolescents and their families.
- Monitor client progress to determine the effectiveness of service delivery and to assess progress towards individual goals and objectives
- Present client cases at weekly multi-disciplinary team meetings to assess needs and develop interventions
- Maintain client information and case documentation in a confidential and professional manner.
- Link clients to the school system for enrollment into high school or continuation programs, junior high school, or post-secondary education/vocational programs
- Provide clients with knowledge about pregnancy, child development, parenting, nutrition, family planning and educational options
- Participate in maintaining a network of services and resources for clients in San Diego County
- Assist with outreach services to identify and bring in eligible clients into the program
- Participate in mandatory program training which includes in-state travel
Skills/Experience/Abilities That Are a Must-Have:
- Bilingual, English/Spanish
- Bachelor’s Degree with a preferred emphasis on child/adolescent development
- 1 year of case management experience, in-home case management experience preferred
- Able to meet case documentation requirements regarding content and timeless
- Understands the importance of, and can preserve, client confidentiality
- Excellent written and oral communication skills
- Sensitivity to cultural differences present in the organization’s/ program’s service population
- Ability to apply good judgment and critical thinking to carry out instructions and make decisions within the scope of authority
- Driver’s License and reliable transportation for regular travel around San Diego County
Skills/Abilities We’d Like You to Have:
- Previous experience working with the adolescent/young adult population
- Knowledge of Early Childhood/Parent/Family Mental Health, Trauma Informed Care, Motivational Interviewing
- Comfort with and discussing and knowledge of sexual health information
Physical Requirements:
- Be seated in a vehicle for at least 1 hour commute around San Diego County
- Safely lift up to 30 lbs
- Speak for at least 1 hour at a time
