Position: Supportive Services Care Manager
Part Time- 20-28 hours/week
Reports to: Supportive Services Supervisor
Salary: Competitive salary based on experience
Overview of Role:
The Supportive Services Care Manager will be part of a supportive, collaborative, and innovative team assisting older adults and enhancing their quality of life through a comprehensive array of services. The Supportive Services Care Manager will work with staff, clients, and community partners to provide digital literacy and programming to older adults. This program looks to engage participants in various platforms to help reduce social isolation and enhance social engagement. The Supportive Services Care Manger will also have a caseload consisting of Kosher Meals on Wheels clients, Assistive Technology clients, and Social Technology clients. They will assist clients with access to ensure their needs are met and will provide advocacy and access to additional resources. The Supportive Services Care Manager will conduct reviews, check in calls, completion of safety assessments, and a nutritional risk assessment including ADL and IADL function. The care manager will assist with other supportive services projects/programs as assigned.

Role & Responsibilities:
- Seek out community partnerships to enhance clientele
- Manage and create programming for online platforms
- Conduct digital literacy trainings for individuals and small groups
- Engage users to utilize technology platforms, ex. Tablet use and online programs
- Conduct in home visits for intakes/reviews or as needed
- Evaluate programs to determine if goals are being met
- Assess the social needs of the older adult community
- Attend weekly supervision
- Comply with standards and requirements of all external accrediting/licensing bodies by attending staff meetings and committee meetings as assigned
- Attend internal/external programs, workshops, supervision, and team meetings
- Maintain client confidence and protects operations by keeping information confidential
- Other duties as assigned
Education/Licensure: BSW or MSW preferred or equivalent education and experience. Older adult experience preferred
