Position: Home Modification Program Manager

Reports to: Director of Supportive Services

Time of Employment: Up to 1,400 hours per year (total 4,200 hours over three years)

Salary: Competitive salary based on experience

Overview of Role:

The Home Modification Program Manager will be part of a supportive, collaborative, and innovative team assisting older adults and enhancing their quality-of-life through home modifications. The Program Manager will work with staff, clients, and community partners to provide home modifications for clients and community members who meet program eligibility. The Program Manager will oversee the program’s day to day operations and follow HUD guidelines for program implementation.

Role & Responsibilities:

  • Develop & complete the construction vendor selection process according to federal requirements
  • Receive referrals from partner agencies and review each client’s eligibility for the project, ensuring that all referrals from partner agencies align with HUD guidelines
  • Provide client referrals to Occupational Therapist (OT) for home assessment and coordinate home visit with clients
  • Provide walk-through for each home modification project, travelling across the four-county area
  • Match clients with appropriate construction vendors
  • Serve as a liaison between clients and construction vendors for each home modification project
  • Attend HUD Project Manager School and other trainings as required
  • Evaluate program to determine if goals are being met
  • Attend internal/external programs, trainings, supervision, and team meetings as needed
  • Maintain client confidence and protect operations by keeping information confidential
  • Other duties as assigned

Minimum Qualifications

Experience: Experience working with older adults, knowledge of community resources and proficiency with technology platforms. Experience with management of home improvement and HUD-funded programs is required.

Education: Bachelor Level
