Description

The Case Manager for the Holocaust Survivor Program will provide the highest quality of care and service to holocaust survivors needing case management services, in collaboration with the Holocaust Survivor Care team. This position involves assessing the needs of the client and providing appropriate resources and support to independence and dignity. The Case Manager will support and collaborate with the Holocaust Survivor Care team to promote motivation and self-sufficiency, as appropriate.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Demonstrates ability to listen, be non-judgmental, and empathetic
  • Assesses new and existing clients for the need of case management
  • Complete needs assessments
  • Provide resources and referrals to help clients maintain independent living
  • Works with clients to achieve financial assistance as needed
  • Maintain compliance with grants and funding sources
  • Document in alignment with appropriate grants and guidelines
  • Visit clients in the field as needed
  • Communicates verbally and in writing in a clear and professional manner
  • Shows initiative and passion for the clients, staff, and work as a whole
  • Acts in a culturally sensitive and PCTI manner at all times

QUALIFICATIONS

  • Experience working with older adults and people experiencing emotional or behavioral issues, cognitive or physical decline.
  • LBSW, LMSW, or case management experience is preferred
  • Ability to convey empathy and non-judgmental caring to all clients and staff
  • Ability to be flexible to handle the versatility of the position and its periodic crises
  • Must work well within a team environment
  • Must be willing to interact with clients off site
  • Must be a self-starter with a willingness to grow clinically and professionally
  • Russian language fluency preferred.

Qualifications
