Description

Salary: $26.00 - $38.00 per hour

This position integrates intake, case management, housing navigation, and payment processing to support homeless individuals with intellectual and developmental disabilities (IDD) in achieving housing stability. Grounded in principles of disability justice and self-determination, this role serves as a central point of contact for clients, partnering with them as experts in their own lives to provide assessments, referrals, and direct assistance. Responsibilities include processing payments on behalf of clients, guiding them through resource navigation, and delivering trauma-informed, person-centered case management services that center dignity, choice, and autonomy. This role requires working with the IDD Housing Navigator to build strong relationships with host home providers, landlords, residential agencies, community partners, and clients while ensuring compliance with program guidelines and documentation standards.

Essential Functions:

  • Practices solidarity with the disability community by promoting self-advocacy, respecting informed choice, challenging ableist barriers within systems, and amplifying client voice in housing and service planning decisions.
  • Provides housing-focused stabilization case management to help clients secure permanent housing and maintain housing stability.
  • Partners with clients on goal setting and acquiring vital documents. Ensures clients remain responsive to client-identified goals and priorities throughout program participation.
  • Conducts client intake and eligibility assessments.
  • Supports clients to navigate and self-advocate through disability determinations and enrolling in Medicaid SLS waiver and Home and Community-Based Services.
  • Works with IDD Housing Navigator to help clients access affordable housing options, mediating with landlords, host home providers, and residential agencies, and applying for housing voucher programs.
  • Connects clients with long-term services and support, including food access, transportation, benefits counseling, employment, recreational, and health services.
  • Provides crisis intervention case management that addresses basic needs and leads to stabilization.
  • Connects clients with employment opportunities and/or income pathways like SSI/SSDI applications.
  • Develops, implements, monitors, and modifies service plans with clients.
  • Develops exit plans and guides clients to transition successfully from the program.
  • Collaborates with other case managers, doctors, organizations, caregivers, family members, etc. to develop creative solutions to complex problems for participants.
  • Models respectful, anti-ableist, and dignified interactions with clients, promoting autonomy, trust, and equitable relationships across each client’s support network.
  • Requests financial approval for services of the program based on assessment of need and availability of resources.
  • Maintains accurate case records in required CRMs; documents client contacts and progress to ensure accountability.
  • Allows new staff and interns to shadow and to sit in with client appointments.
  • Works to ensure proper completion of paperwork and adherence to grant eligibility.
  • Assists with reporting as needed by providing client testimonies.
  • Collaborates with community partner agencies.
  • Other duties as assigned.
Requirements

Education/Experience:

  • Preferred three plus years experience as a Social Case Worker for a public or private agency.
  • Preferred one plus years experience working with adults with IDD.

  • Bilingual in Spanish (required)
  • Demonstrated commitment to disability inclusion, equity, and person-centered practice; lived experience or strong allyship with the disability community is valued.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to interact effectively as a team member and independently with agency staff and with a diverse client base.
  • Ability to navigate effectively in a fast-paced environment.
  • Must be proficient in Microsoft Office Software.
  • Knowledge and understanding of the Denver metro area resources is preferred.

Work Environment:

Off-Site & In-Office Position: Work primarily takes place in the community with routine requirements to be at the home office at 3201 South Tamarac Dr., Denver 80231. Work hours are generally during regular business hours, with occasional flexibility required to support agency needs.

Physical Requirements: Frequent sitting and typing. While performing the duties of this job, the employee is regularly required to use standard office equipment. The employee is regularly required to travel between sites (with and without clients), communicate effectively, and use standard mobile equipment. The employee may occasionally be required to lift up to 10 pounds.
