Description

Salary Range: $24.00 - $29.00 per hour

Position Summary:

Coordinates expansion and delivery of emergency assistance and care management services to individuals and families in need, and community groups such as caregiver support and grief groups. Performs administrative and data tracking duties to support the Boulder JFS team.

Essential Functions*:

  • Engages in the strategic growth of the Boulder office with a specific focus on emergency assistance and care management program expansion.
  • Conducts client care management virtually or in-person.
  • In consultation with agency staff, develops, maintains, and supports the emergency assistance program.
  • Engages with landlords, mortgage companies, etc., to collect needed documentation with sensitivity to confidential information.
  • Provides information and referrals to clients.
  • Conducts client intake and eligibility assessments and reassessments to identify client support needs.
  • Creates and maintains detailed client profiles in the electronic client database and in SharePoint.
  • In consultation with agency staff, coordinate community groups across Boulder, organize and deploy caregiver support groups and grief groups at multiple community locations, often occurring simultaneously during a short time period.
  • Communicates and maintains positive relationships with assisted living/residential facility/community center staff.
  • Collects pre- and post-surveys for support groups.
  • Communicates about groups, such as writing up group descriptions for social media and other promotional materials, coordinating with the JFS Marketing team.
  • Works to ensure proper completion of paperwork and adherence to grant eligibility and data tracking.
  • Represents JFS at various community outreach and educational events as needed.
  • Answers questions and provides linkages to various benefit programs and/or options for services that best meet the needs of clients and/or family members.
  • Attends professional trainings, seminars, in-services, and other staff development opportunities.
  • Actively participates in department and agency meetings in both Boulder and Denver locations, as well as ad hoc committees, as requested by the director or CEO
  • Oversee the Boulder mainline voicemails and provide information, referrals, and information and assistance, as appropriate.
  • Expand outreach to additional referral sources, including physical therapists, gerontologists, Council on Aging, faith communities, and other community partners aligned with the target population.
  • Work with the Director of Boulder and the Development and Community Engagement Manager to launch a community awareness and outreach plan that includes presentations, small gatherings, tabling events, and intentional word-of-mouth engagement.

Requirements

Education/Experience:

  • Required bachelor's degree in social work, related field, or equivalent work experience.
  • Preferred three plus years’ experience as a Social Case Worker for a public or private agency.
  • Preferred two or more years of experience coordinating programs or a customer service-related position.
  • Preferred two or more years of experience working with older adults, adults?navigating chronic health challenges, and family members?impacted?by these populations.
  • Preferred two or more years of group facilitation experience.

Other:

  • Ability to provide good client and customer service. This job requires being pleasant with others on the job and displaying a cooperative attitude while being careful about detail and thorough in completing work tasks.
  • Substantial working knowledge of Microsoft Office.
  • Understanding of Jewish community and traditions preferred.
  • Non-profit experience preferred.
  • Excellent written and verbal communication skills including phone and in-person interviewing.
  • Able to manage time effectively and work independently while organizing workload.
  • Sensitivity and discretion regarding personal/confidential information.
  • Able to comfortably engage with a diverse range of people.
  • Able to track and input information and documents into electronic client databases.

  • None.

Supervisory Responsibilities:

  • None.

Work Environment:

Hybrid Position:

  • Work is primarily performed in our office at 6007 Oreg Avenue, Boulder, CO 80303.
  • Work hours are generally during regular business hours, with occasional flexibility required to support agency needs.

Essential physical requirements:

  • Frequently sitting and typing. While performing the duties of this job, the employee is regularly required to communicate and use standard office equipment.

Physical effort/lifting:

  • The employee may occasionally be required to lift up to 10 pounds.
