Position Summary: This member of the Aging Care and Connections Department will provide care management services to older adults, adults with disabilities, and family members impacted by these populations. This position will also oversee the day-to-day operations of the Kosher Congregate Meal site at the JCC and the Friendly Visitor and Senior Companion programs.

Colorado residency is required.

Hiring Range $26.00 - 30.00 per hour

Responsibilities:

A. Care Management:

a. Completes initial and ongoing assessments/reassessments to identify client service and support needs.

b. Provides education on and linkages to various local or state benefit programs and/or options for services that best meet the needs of the client and/or family members.

c. Provides ongoing service coordination including, but not limited to referrals, advocacy, and mediation with the client and community service providers.

d. Provides ongoing service monitoring by evaluating the quality, quantity, and duration of service provided by JFS and/or external providers.

e. Occasionally provides direct assistance with transportation to appointments requiring care manager presence/advocacy.

f. Participates in care management office hours at off-site partner locations

g. Monitors client satisfaction with services and adjusts interventions as needed.

h. Establishes professional and effective communication and collaboration with client and others involved in their care.

i. Articulates current and future service benefits, outcomes, and costs clearly to clients and/or family members.

j. Responsible and accountable for accurate and timely completion of all applicable forms, data entry, statistics tracking, and documentation of care management activities.

B. Kosher Congregate Meal Site:

a. Occasional setup for the Kosher meal service at the Jewish Community Center, including pre-cleaning, setting the tables, and making coffee.

b. Ensure food is picked up for Shalom Park, delivered to the JCC, and Cambros are returned to Shalom Park

c. Ensure staff and volunteers are available to serve food to program participants and clean up following the meal

C. Friendly Visitor Program and Senior Companion Program

a. Assist care managers with Friendly Visitor referrals

b. Assess caseload of Friendly Visitor Clients

c. Manage Senior Companion program caseload

d. Submit necessary Senior Companion paperwork to Spark the Change

D. Attends internal or external meetings, as needed, to advocate for client wishes/needs.

E. Maintains knowledge of agency-wide policies, department procedures, and funding-specific regulations.

F. Attend professional seminars, trainings, and in-services

G. Provides technical assistance and general support to department volunteers and student interns.

H. Provides support to agency-wide central intake as needed.

I. Actively participate in department and agency meetings as well as ad hoc committees, as requested by the supervisor or director.

J. Designs and executes educational presentations to community stakeholders and JFS staff, as requested. Represents JFS at community outreach events.

K. Other duties as assigned.

Requirements

Qualifications:

  • Effective interpersonal communication skills, knowledge of resources, and available interventions for the population served.
  • Strong organizational and time management skills.
  • Basic computer skills and experience with Microsoft Office products.
  • Familiarity with client database systems.
  • Ability to work autonomously.
  • Comfortable conducting face-to-face assessments in client homes.
  • Must be a licensed driver with an insured vehicle.

Education:

  • Master’s or bachelor’s degree in human services or related field.

Experience:

  • Experience and education in gerontology or social work. 2 -3 years working with older adults and/or adults with chronic illness. Internship experiences will be considered.
