Job Title: Aging Services Clinical Case Manager

Accountability: Director of Program Quality Assurance/Clinical Supervisor

Qualifications:

  • Excellent grasp of clinical theory and application of evidence-based practices in mental health and substance abuse issues affecting the geriatric population.
  • Minimum of 2 years’ experience in social work or closely related field.
  • Florida license eligible.

Summary of Position:

Provide individual support and ensure comprehensive delivery of services to program participants and callers to agency.

  • Conducts intake/needs assessment with new clients or existing clients seeking case management.
  • Coordinates care to Aging services clients with community partners and resources identified from the needs assessment.
  • Advocates on behalf of clients for services, basic needs, and other related issues.
  • Navigation and connection of clients and families to needed resources.
  • Stays abreast of community resources and agency programs.
  • Returns phone calls within a timely manner, usually 24 hours.
  • Other duties as assigned

Counseling Services

Provide outpatient therapy services to individuals, groups and families in an office setting, off-site in the community, in-home and telehealth when appropriate.

  • Assist with conducting therapeutic groups at partner agencies
  • Organize, develop and complete bio-psychosocial assessments, treatment plans and progress notes.
  • Return phone calls in a timely manner, usually within 24 hours.
  • Meet agency’s productivity requirements as determined by supervisor.
  • Attend all clinical meetings as directed by supervisor.
  • Other duties as assigned.

Education and Experience:

  • Masters in Mental Health Counseling or Masters in Social Work from an accredited academic institution. Registered Intern preferred.
  • At least 2 years’ experience in social case work or closely related work
  • Excellent written and oral communication skills
  • Experience working with aging population and diverse populations and backgrounds
  • Well organized with excellent time management skills
  • Understand and support cultural diversity
  • Bilingual (English/Spanish) a plus

Required Skills/Areas of Accountability:

  • Ability to engage and work with the aging population
  • Ability to work independently
  • Capacity to understand the obligations, functions, programs and services of the agency and refer clients to other interagency programs and services within the community as appropriate.
  • Ongoing interest in obtaining training to update skills and treatment interventions especially in the high-risk area of seniors and vulnerable adults.
  • Strong ability to relate to other professionals and lay people in the community regarding JFCS, its mission, vision and programs.

Training Requirements:

  • Annual HIPAA training
  • Annual Deaf & Hard of Hearing Training
  • All other annual required agency wide and counseling specific trainings
