Registered Dietitian – L’Chaim Adult Day Health Care

L’Chaim Adult Day Health Center is a licensed facility and outpatient center that provides medical, social, and therapy services for Russian-speaking older adults and individuals with disabilities. We have been a haven of person-centered care and community support for thousands of emigre clients from San Francisco and San Mateo for over 25 years.

POSITION SUMMARY:

We are seeking a part-time Registered Dietitian to join our interdisciplinary care team. In this role, you will provide nutrition assessments, develop individualized care plans, and oversee food service operations in compliance with CACFP and Title 22 regulations. You will also have the opportunity to deliver engaging nutrition education and work closely with our staff and families to support participant well-being. COMPENSATION AND BENEFITS:

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Conduct initial nutrition assessments and six-month reassessments for all participants; update individualized care plans
  • Collaborate with nurses, physicians, and interdisciplinary teams to address participant dietary needs
  • Document all assessments, care plans, and changes in compliance with Title 22 regulations
  • Monitor significant changes in participant nutritional status and adjust plans as needed
  • Oversee food service operations, including monthly caterer site inspections and menu reviews
  • Verify therapeutic diets, substitutions, and cultural or linguistic dietary needs
  • Perform food quality and safety checks to ensure CACFP compliance
  • Provide quarterly nutrition education classes and individualized counseling to participants
  • Deliver annual nutrition and food safety training to program staff
  • Foster strong relationships with participants, caregivers, and community partners to ensure satisfaction and continuity of care

REQUIREMENTS:

  • Registered Dietitian (RD) or RD eligible; bachelor’s degree in nutrition required; master’s degree preferred
  • 2+ years’ experience with older adults or community health programs as a Dietitian, conducting nutritional assessments, coaching, and counseling clients
  • Current BLS and First Aid certification required
  • Previous experience in CACFP administrative reviews for program compliance is required
  • Bilingual in Russian is required
  • Ability to lift up to 50 lbs and support frail or disabled adults
  • Proficiency in Microsoft Office and Electronic Health Records
  • Ability to travel to catering sites for inspections
