Bilingual Therapeutic Mentoring Case Manager

The Therapeutic Mentoring Case manager provides individualized support to students to promote school attendance, engagement, academic success, and social-emotional well-being. Using the Check & Connect model, this position focuses on relationship-building, mentoring, and advocacy to help students overcome barriers to success. The case worker will work in school settings, community sites, and homes within Baltimore County to provide consistent mentorship and guidance.
Essential Duties and Responsibilities
Student Mentorship & Engagement
- Develop and maintain positive, supportive relationships with students to promote school attendance, academic achievement, and personal growth.
- Utilize the Check & Connect model to monitor student progress, encourage goal-setting, and implement strategies for success.
- Provide individualized support to address challenges related to attendance, academics, and social-emotional development.
Collaboration with Schools and Community
- Work within one or more school sites, coordinating with teachers, counselors, and administrators to build referrals, maintain caseload and support student success.
- Engage with families by phone, through in-person visits in their homes and community settings as needed to provide holistic support and strengthen school-home connections.
- Serve as a liaison between students, families, schools, and community resources, advocating for student needs and ensuring access to appropriate support services.
Progress Monitoring and Documentation
- Track student attendance, behavior, and academic performance using structured monitoring tools.
- Maintain accurate records of student interactions, progress, and interventions in accordance with program guidelines.
- Participate in team meetings, interdisciplinary school meetings, and case reviews to ensure alignment with student goals and program objectives.
Grant Compliance and Program Requirements
- Adhere to all grant guidelines and reporting requirements, ensuring program fidelity and accountability.
- Participate in training and professional development related to Check & Connect and best practices in mentoring and student engagement.
- Provide regular updates and reports on student progress to program leadership and school personnel as required.
Qualifications and Requirements
- No degree or specific licensure required; experience working with adolescents in educational, mentoring, or community settings preferred.
- Ability to build strong, trusting relationships with students and families from diverse backgrounds.
- Strong communication and collaboration skills, with the ability to work effectively with school staff, families, and community partners.
- Comfort with working in school, home, and community settings within Baltimore County.
- Reliable transportation and willingness to travel between multiple locations as needed.
- Bilingual in Spanish is preferred.
Work Environment
- School and community-based role with regular travel required.
- Position is grant-funded and contingent on continued funding availability.
