π What You'll Do (And Love!)
π€ Provide Direct Case Management
- Conduct assessments: Evaluate residents' needs, strengths, and goals for aging in place
- Develop care plans: Create individualized support strategies in partnership with residents
- Coordinate services: Connect residents to JCS internal programs and external community resources
- Advocate for residents: Navigate systems, resolve barriers, and ensure access to benefits and services
- Monitor progress: Track outcomes and adjust care plans as needs evolve
π Support Community Living
- Facilitate smooth transitions: Help new residents integrate into the community
- Address conflicts: Use compassionate communication and restorative practices for conflict resolution
- Build relationships: Foster trust through consistent, respectful interactions
- Identify service gaps: Recognize unmet needs and work to address them
- Support crisis intervention: Respond to emergencies and connect residents to appropriate resources
π Ensure Compliance & Documentation
- Maintain accurate records: Document all resident interactions and service coordination per HUD guidelines
- Track service delivery: Monitor and report on case management activities and outcomes
- Ensure fair housing compliance: Uphold HUD standards and fair housing principles in all services
- Collaborate with housing staff: Align case management with property management and occupancy needs
- Participate in care team meetings: Work with SASH coordinators, social workers, and other JCS staff

- Manage assigned caseload: Provide ongoing case management services to designated residents
- Prepare reports: Document service statistics, outcomes, and resident feedback
- Support program development: Contribute to service planning and quality improvement initiatives
- Grant writing support: Assist with funding applications to support resident services
- Resource development: Build and maintain community partnerships and referral networks

Qualifications
β¨ What We're Looking For
Education & Experience
- Bachelor's degree required in social work, human services, gerontology, or related field
- 2+ years experience working with older adults in residential, case management, or program settings
- Experience with service coordination, advocacy, or care management preferred
- Knowledge of aging services, benefits systems, and community resources
Essential Skills
- Strong assessment and planning abilities: Skilled in evaluating needs and developing care plans
- Excellent communication: Ability to engage compassionately with older adults and diverse populations
- Conflict resolution expertise: Active listening and mediation skills for addressing resident concerns
- Systems navigation: Knowledge of benefits, healthcare, and community service systems
- Documentation proficiency: Strong written communication and record-keeping skills
- Cultural competency: Ability to work respectfully with people from diverse backgrounds
Technical Requirements
- Computer proficiency: Microsoft Office, email, database systems
- HUD knowledge: Familiarity with HUD housing principles and fair housing laws preferred
- Case management systems: Willingness to learn specialized tracking and documentation platforms
- Crisis intervention: Ability to respond appropriately to emergencies and challenging situations
