Jewish Collaborative Services (hereafter known as JCS) was established in 2018 because of a combination of five prior entities (Jewish Family Service, Jewish Seniors Agency, Shalom Apartments, Shalom II Apartments, and Tamarisk Senior Living). The combined agency's mission is to provide a continuum of supportive services, guided by Jewish values, that strengthen the well-being of our community members. JCS strives to serve people of all faiths with better access to professional services in the greater Rhode Island and nearby Massachusetts communities. The emphasis for the new agency will be in recognizing and meeting current and emerging community needs within the framework of Jewish traditions and values into action. Position Title: Wellness Nurse

FLSA Status: Non-exempt

Schedule: Full-time, with rotating weekends, and on-call requirements

Position Location: Tamarisk Senior Living, Warwick, RI

Job summary: The Wellness Nurse plays a vital role in promoting the health, safety, and well-being of our residents. This position is ideal for someone who is passionate about enhancing the quality of life for older adults and thrives in a collaborative, mission-driven environment. The Wellness Nurse is also an essential support person in our wellness department assisting with the daily management of the residents’ nursing needs and oversight, education, and support of our direct care staff.

Supervisory Responsibilities:

  • Works closely with caregiving staff to oversee day-to-day operations while ensuring high standards of care. Provides role modeling and instruction as needed.

Duties/Responsibilities:

  • Supports nursing care standards through adherence to existing policies and procedures.

  • Ensure accurate administration and documentation of medications as prescribed by healthcare providers.

  • Educate residents, staff, and families on wellness topics, including healthy aging, chronic illness prevention, and personal care techniques.

  • Regularly attends unit conferences and makes recommendations for improvement in patient care via the patient care conference and unit conference.

  • Participates in peer review as appropriate.

  • Obtains and documents nursing history from patients and/or significant others that is comprehensive of present condition and prior problems.

  • In partnership with and under the supervision of an RN, performs basic assessment of patients, indicating adequate knowledge of anatomy, physiology, and pathology. Records vital signs, note changes, and pursues more-specific investigations as needed.

  • Identifies abnormal diagnostic data and shows a fundamental understanding of its implications by reporting information to the physician and others caring for the patient, and by incorporating information in the nursing care plan.

  • In collaboration with and under the supervision of an RN, uses assessment information to develop a care plan before the end of duty time that communicates enough information for incoming staff to adequately care for the patient.

  • Reviews and develops care plans for assigned patients. Includes the patient and their family in planning care, establishing priorities, and setting goals.

  • Uses care plans as a basis for providing safe and therapeutic care to patients.

  • Communicates with the patient and their family and other members of the health care team in carrying out the goals of the care plan.

  • Accepts accountability for clinical care of assigned patients including supervision of professional and nonprofessional personnel.

  • Evaluates and notes the patient's response to nursing care defined in the nursing care plan and medical records.

  • Assisting residents with ADL's according to resident need.

  • Administering medications and performing treatments per physician orders.

  • Respecting resident's rights and maintains resident confidentiality.

  • Assumes responsibility for professional growth by current reading and attendance at educational programs and unit conferences. Attends in-services and meetings as required by the state to maintain current certification.

  • Participates in rotational weekend coverage on a biweekly basis.

  • Performs other duties as assigned.
