Job Summary:

The Health Home Care Manager (HHCM) Supervisor ensures the care management team effectively delivers services in accordance with Health Home Core Services. This role supports staff in addressing barriers to care—such as financial hardship, housing, transportation, social support, and health literacy—through clinical guidance and administrative oversight. The Supervisor monitors the referral and enrollment process, ensures timely documentation and quality service delivery, and promotes a coordinated, member-centered approach. They serve as a resource for HHCMs, fostering accountability, compliance, and continuous improvement in care coordination.

Key Responsibilities:

  • Supervise HHCMs in delivering Health Home’s six core services based on member acuity.
  • Ensure timely face-to-face contacts, documentation (POC, CANS, progress notes), and compliance with Health Home IT systems.
  • Maintain adherence to federal, state, and city regulations and conduct regular audits for quality assurance.
  • Provide clinical guidance on motivational interviewing, member engagement, and self-management.
  • Assign and monitor caseloads to ensure service provision and billing accuracy.
  • Coordinate with finance on claims and reconciliations.
  • Support team coverage needs and collaborate with stakeholders to assess member service needs.
  • Maintain program census, including intake, enrollment, and discharges.
  • Attend meetings and trainings, and oversee hiring, evaluations, and staff performance.
  • Represent the program internally and externally, and ensure coordination with Medicaid MCOs, SPA, and HCBS providers.
  • Participate in policy development, quality improvement, and after-hours coverage as needed.

Qualifications:

  • Required:
    • Master’s Degree (Social Work, Psychology, Health/Public Administration) or RN with 2+ years’ relevant supervisory experience.
    • Experience with service coordination for individuals with disabilities, complex trauma, or special needs.
    • Strong organizational, communication, and computer skills.
    • Ability to work with diverse teams and travel within NYS.
    • Confidentiality and adaptability to program needs.
  • Preferred:
    • Bilingual (Spanish)
    • Knowledge of person-centered planning, cultural competency, LGBTQ+ issues
    • Experience with community safety and outreach

Salary Range: $66,300 - $71,500
