Job Summary:
The Health Home Program at JCCA is a care management program for children with chronic conditions. The Health Home team members connect all of the members of the child's treatment team and assist family members in accessing community resources to improve health outcomes and the child's wellbeing.
JCCA is seeking an Assistant Director of Health Homes to provide assistance in delivering administrative supervision, management and support to the health home program director and program staff to ensure that quality programming and services are provided to meet the medical, behavioral, social, and developmental needs of the members served in the Health Home program. The AD will oversee staff in both the Bronx and Brooklyn offices and should be flexible to travel to each office.
*Primary location of this position can be the Bronx or Brooklyn.
Summary of Tasks:
- Assist the Director in monitoring program census and enrollment trends to support growth, capacity planning, and program development initiatives.
- Provide ongoing guidance, support, and as-needed individual and group supervision to Health Home Care Manager (HHCM) Supervisors to promote effective service delivery and staff development.
- Monitor service provision and documentation to ensure timely and accurate billing.
- Maintain comprehensive knowledge of Health Home requirements, Children and Family Treatment and Support Services (CFTSS), Home and Community-Based Services (HCBS), Managed Care, and other applicable program regulations.
- Stay current on New York State Department of Health (DOH) regulations and policy updates affecting program operations.
- Address operational issues as directed by the Director or Behavioral Health Services Assistant Vice President (BHW-AVP); communicate regulatory updates to staff and review electronic health records (EHRs) and documentation to ensure compliance with regulatory, contractual, and organizational standards.
- Assist in the coordination, management, and completion of internal and external audits.
- Partner with the Director, Quality Management, and Compliance teams to strengthen service delivery, improve client outcomes, and support continuous quality improvement efforts.
- Collaborate with the Director to plan and coordinate staff development initiatives, in-service trainings, and professional learning opportunities.
- Oversee staff training compliance and ensure completion of all required trainings and certifications.
- Facilitate, coordinate, and manage program committees and interdisciplinary review (IR) processes.
- Provide direct supervision and support to the Program Support Specialist.
- Manage and monitor wraparound service expenditures to ensure appropriate utilization of program resources.
- Represent the program by participating in Lead Health Home (CBC and CCF) virtual office hours, workgroups, and monthly meetings, ensuring communication of updates and implementation of best practices.
Qualifications:
- Master’s degree in Social Work, Psychology, Public Administration, Healthcare Administration, Hospital Administration, or a related field required.
- Minimum of two (2) years of relevant experience, including supervisory responsibility within a mental health, behavioral health, medical health, or human services program.
- Demonstrated experience providing service coordination, care management, information and referral services, and community-based linkages for children and families with complex needs, including those affected by trauma, special healthcare needs, and/or developmental or physical disabilities.
- Strong organizational, time management, and follow-through skills, with the ability to effectively manage multiple priorities and deadlines.
- Proficient in the use of technology and able to quickly learn and adapt to new software systems, electronic health records (EHRs), and other information technology platforms.
- Proven ability to build effective working relationships and collaborate with diverse individuals, teams, and community partners.
- Demonstrated commitment to maintaining confidentiality and handling sensitive information with discretion and professionalism.
- Flexible and adaptable, with the ability to respond effectively to changing program needs, priorities, and regulatory requirements.
- Excellent verbal and written communication skills, with the ability to communicate clearly, professionally, and effectively across all levels of the organization.
