JCCA is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to provide high-quality, comprehensive medical care to children and adolescents involved in the child welfare system. Working in one of New York's most respected residential treatment programs for youth in foster care, you will play a vital role in promoting the physical and emotional well-being of children.

In this position, the FNP will perform assessments, diagnose and manage acute and chronic conditions, provide immunizations, and deliver preventive health services. The FNP collaborate closely with a multidisciplinary team to ensure each child receives coordinated, trauma-informed care.

Summary of Responsibilities:

  • Provide direct primary care to clients ages 6 to 21, developing therapeutic care plans focused on prevention, management, and the promotion of optimal health for children in care.
  • Assess, diagnose, treat, and manage both acute and chronic health conditions, ensuring comprehensive and continuous care.
  • Deliver preventive health and family planning services.
  • Order, perform, interpret, and evaluate diagnostic tests and procedures, ensuring timely follow-up and care coordination.
  • Prescribe medications, monitor treatment outcomes, and adjust therapeutic plans as appropriate.
  • Coordinate referrals to healthcare professionals, specialists, and community clinics, and support staff in building and maintaining partnerships with community providers.
  • Provide guidance and instruction to other healthcare staff in client care management.
  • Conduct staff and client training sessions as needed.
  • Ensure compliance with HIPAA and all applicable privacy and confidentiality standards.
  • Collaborate closely with JCCA’s pediatricians and multidisciplinary care teams to ensure integrated, trauma-informed care.

Qualifications:

  • Master’s or degree in Nursing from an accredited Nurse Practitioner program.
  • Current New York State license as a Registered Professional Nurse and certification as a Family Nurse Practitioner (FNP).
  • Board certification as a Family Nurse Practitioner (ANCC or AANP).
  • Experience providing comprehensive primary care to children and adolescents, ideally in a residential treatment setting.
  • Familiarity with trauma-informed care and experience working with populations impacted by trauma, abuse, or neglect.
  • Excellent interpersonal and communication skills, with the ability to collaborate effectively within a multidisciplinary team.
  • Demonstrated empathy, professionalism, and a strong commitment to improving the health and well-being of children and families.

Salary: $125K - $135K
