Position Summary: The Case Planning Supervisor for the Medically Fragile Program plays a critical leadership role in ensuring the health, safety, and well-being of children with complex medical needs placed in foster homes. This role provides both clinical and administrative supervision to a multidisciplinary team—including case planners, nurses, and socio-therapists—to ensure high-quality, trauma-informed, and coordinated care. The Supervisor collaborates closely with medical providers, foster parents, birth families, ACS, and internal teams to ensure that all services meet regulatory requirements, medical standards, and the unique needs of each child.

Key Responsibilities

Program & Clinical Supervision

  • Provide clinical and administrative supervision to Case Planners and Socio-Therapists.
  • Oversee casework assignments and service planning; ensure compliance with timelines and documentation requirements.
  • Conduct regular individual and group supervision, team meetings, and case conferences.
  • Ensure timely completion of assessments, service plans, and permanency planning in line with ACS and agency standards.
  • Deliver reflective supervision to strengthen clinical practice and trauma-informed care.

Medical & Service Coordination

  • Oversee coordination of medical care, therapy, equipment, and educational services for medically fragile children.
  • Monitor completion of medical consents, follow-ups, and regulatory documentation.
  • Serve as liaison between medical providers, ACS, and other service partners.
  • Partner with foster and birth families to support collaborative care and permanency goals.
  • Participate in Family Team Conferences, court hearings, and case reviews as needed.

Compliance & Quality Assurance

  • Ensure team compliance with ACS, OCFS, and state regulations.
  • Maintain accurate, timely documentation in CONNECTIONS and other case management systems.
  • Complete required supervisory notes and reporting (e.g., Shared Response, OCFS/ACS data spreadsheets).
  • Collaborate with internal teams on compliance, nursing, and quality improvement initiatives.

Leadership & Administrative Functions

  • Provide leadership, guidance, and support to direct practice staff.
  • Participate in staff hiring, onboarding, training, performance reviews, and progressive discipline.
  • Approve schedules, time off, and overtime in accordance with agency policies.
  • Provide backup administrative coverage during staff absences.
  • Help develop and implement program policies, procedures, and standards.
  • Cover caseloads in the absence of case planning staff when needed.

Qualifications

  • Master’s degree in Social Work, Public Administration, or a related field required.
  • Licensed or license-eligible strongly preferred.
  • Minimum of three (3) years’ experience in child welfare, foster care, or related health/mental health services.
  • At least one (1) year of supervisory or leadership experience preferred.
  • Strong knowledge of ACS/OCFS regulations and working with medically fragile populations.
  • Excellent organizational, communication, and crisis management skills.
  • Ability to collaborate with diverse teams and stakeholders.
  • Proficient in CONNECTIONS, Microsoft Office, and electronic case record systems.
  • Understanding of institutional and structural racism and its impact on families and communities.
  • Bilingual (English/Spanish) preferred.

Work Schedule & Coverage

  • Must be available for 1–2 late nights per week.
  • Participate in after-hours and weekend administrative coverage on a rotating basis.

Salary Range: $67,000 – $83,000 annually (commensurate with experience and licensure)
