Summary
JCCA’s Home-Based Community Intervention (HCBI) program supports youth (ages 5–20) transitioning from inpatient or residential care, or at risk of placement. Our family-driven, youth-guided model brings crisis intervention, stabilization, and skill-building directly to homes, schools, and communities. HBCI utilizes a number of interventions, including: the active participation of the family and other natural supports, the utilization of promising practices and evidence-based treatment interventions focused on family and systems approaches, meaningful connections within the home and community, and preparations for transition to adulthood; all as applicable to the population served.
As an HCBI Therapist at JCCA, you will play a vital role in supporting children and families through tailored interventions. This position is crucial in helping young people develop the skills they need for productive and independent lives.
Responsibilities
- Deliver short-term, evidence-based therapy and crisis support to youth and families
- Conduct assessments, safety planning, and case management
- Provide “warm hand-offs” to ongoing services and ensure a safe home environment
- Adapt intensity of services based on evolving family needs
- Document care, participate in team supervision, and maintain a community caseload
- Monitor progress and adjust interventions as necessary to ensure effectiveness.
- Share on-call responsibilities with the Supervisor and other Interventionists on a weekly, rotating basis, where the Interventionists respond to crisis calls remotely after hours and on weekends. Additional compensation provided for on-call rotations.
Qualifications:
- Must hold a valid New York State license: LMSW, LMFT, LMHC, or LCAT; limited permit is acceptable at time of hire
- Experience working with youth with serious emotional disorders, developmental disabilities (IDD), or complex behavioral health needs.
- Strong clinical skills, particularly in family systems, crisis intervention, and trauma-informed care.
- Familiarity with evidence-based treatment models.
- Spanish language skills are a strong plus.
Additional Skills:
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
- Excellent interpersonal, communication, and organizational abilities.
- Knowledge of Social Determinants of Health, racial equity, and social justice.
- Capacity to work collaboratively with cross-functional teams.

Salary: $62,400 - $64,920 - differential offered for Spanish language skills and/or NYS LMSW, LMFT, LMHC, LCSW or LCAT.
