Position Summary: The Family Team Conference Facilitator Level I is responsible for facilitating and tracking all conferences as required by the Administration for Children’s Services (ACS) utilizing the Family Team Conference Model. The facilitator will ensure the safety and well-being of each family is assessed through a strength-based lens. Utilizing strong engagement skills, the facilitator will yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders. Responsibilities include but are not limited to the following:
Responsibilities:
- Facilitate all Family Team Conferences types including, Service Termination and 30-45 Conferences;
- Maintain fidelity to the Family Team Conference Model;
- Track activities related to FTC conferences including timeliness and compliance;
- Participate in assigned preventive model training, demonstrate knowledge of model and support model fidelity;
- Collaborate and strengthen partnerships with community representatives to ensure participation and involvement in conferences;
- Maintain an active and flexible FTC schedule;
- Schedule conferences and disseminate notifications to all participants;
- Conduct visits to participants homes in order to hold conferences when needed;
- Hold pre-conferences and debriefings;
- Participate in individual and group supervision, staff meetings, trainings and planning sessions as appropriate;
- Conduct follow-up reviews to ensure progress with identified task;
- Serve as an active member of the interdisciplinary team including attending case conferences, reviewing each family’s case record in preparation for the FTC and maintaining communication with team members during the life of the case to support the integration of the program model;
- Assess the strengths and needs of each child and family member in consultation with the interdisciplinary team and specialty consultants;
- Identify, evaluate and address any and all safety and risk concerns during conferences;
- Complete accurate and timely documentation in Connections, PROMIS and related agency databases;
- Assist in the completion of all FTC’s related reports;
- Ensure adherence with agency, ACS and OCFS policies and procedures;
- Perform other related duties as assigned by supervisor.
Qualifications:
- Bachelors in Social Work or closely related field from an accredited college or university.
- Must be proficient in Microsoft Excel and Word
- Exceptional mediation skills to effectively facilitate group conference situations.
- Strong writing skills.
- Excellent organizational skills
- Ability to handle multiple tasks at one time.
- Have a focus on outcomes and results
- Knowledge of core principles and standards for NYC Child Welfare system.
- Ability to travel in and around New York City region
- Willing to work a flexible schedule that includes evenings and at least one Saturday a month.
Preferred Experience:
Preferred candidate will have at least two years of experience working in the child welfare system, FTC trained or familiar with group conferencing process. Excellent interpersonal skills, ability to work in a team environment and lead diverse groups toward resolution of differences.

Salary: $49,500 - $51,500
