OBJECTIVE: Work in partnership with Social Worker and treatment team to assure a safe and permanent outcome for every child.
ESSENTIAL TASKS:
- Act as liaison with ACS, DSS, CSE, and OCFS workers and as well as officers of the court.
- Represent team in Family Court Hearings and bring feedback from court to team for follow-up.
- Participate in Family Team Conferences and Initial Treatment Conferences.
- Participate in team briefings and document these briefings in Connections.
- Identify cases that require and participate in Planning & Service and Red Flag meetings.
- Assist with supervising family and/or sibling visits when necessary.
- Assist with home visits when necessary.
- Assist in developing visiting plans with families.
- Crisis intervention as needed.
- Collaborate with team members to generate individualized case management, treatment and discharge plans.
- Responsible for providing referrals to family and youth for treatment and case management purposes.
- Responsible for arranging, participating, and documenting meetings at school at least two times per academic year.
- Implementation of AWOL protocol including notifying ACS/DSS/CSE via phone and progress note, notifying FCLS Attorney, obtaining and vacating warrant, ensuring that Missing Person’s Report is filed and vacated upon child’s return.
- Refer and follow-up with community resources as needed.
- Complete FASPs, Court reports, TPR summaries, and surrender applications.
- Maintain progress notes.
- Transfer incident reports into Connections progress notes.
- Write Transfer Summaries.
- Complete of Statistic Change Forms and CONX notes documenting movements of children.
- Other documentation as needed.
- Participate in special events.
- Hours must be flexible. Expectations are that case manager is available for evening and weekend work at least 2 nights per week.
JOB SPECIFICATIONS:
- BSW or BA
- Demonstrated ability to collaborate with client and teams that are culturally and professionally diverse.
- Computer Literacy (specifically Microsoft Word, Excel). Ability to navigate NYS Connections system.
- Solid organizational and follow-up skills
- Excellent telephone and interpersonal skills
- Excellent writing skills.
- Bilingual/Spanish preferred
Salary range:
Bachelors Degree - $48K -$51K
Masters Degree - $56k - $63k
