Case Manager (UC)

Position Summary: Case Manager, who is responsible for assessing the needs of each UC in care, developing an Individual Service Plan, screening for human trafficking concerns, facilitating the timely release or discharge of the UC, and documenting the provision of services in each UC's case file. Case Workers maintain direct contact with each UC and, to the extent possible, with the UC's family and potential sponsors. All Case Worker staff are required to be computer proficient.
Responsibilities:
· Weekly and monthly face to face contact with assigned UCs
· Lead monthly staffing with Case Coordinator and quarterly staffing with FFS (Federal Field Specialist)
· Complete progress notes and SIRs as required
· Complete and follow-up with Educational Planning and school visits as needed
· Oversee search of viable sponsors and follow-up as needed
· Accompany UC to Immigration Court and facilitate meeting with Immigration Attorney as needed
· Liaise with Child Advocate
· Complete SIRs as needed
· Complete ISP (Individual Service Plan), Case Reviews, Risk Assessments and UC Assessments as needed
· Develop Behavior Modification Plans with UC and foster parent.
· Monitor foster homes to ensure compliance
· Collaborate in leading foster parent training groups
· Collaborate with staff in creating and facilitating recreational activities
· Provide On-Call Support as required or scheduled
· Assist UC program wherever needed
Required Qualifications:
· Bachelor’s degree from an Accredited University
· Experience working with children and families or unaccompanied minors, clinical work with minors, knowledge of trauma and its implications.
· Ability to work independently and also in a team setting.
· Bi-lingual (English and Spanish)

Range: $26.68 to $ 28.08 per hour
