See open jobs at Jewish Child Care Association (JCCA).See open jobs similar to "Family Team Conference Facilitator Level II" CareerHub.Position Summary: Under the supervision of the Director of Conferencing or Conference Manager, the Family Team Conference (FTC) Facilitator is responsible for facilitating and tracking all conferences as required by the Administration for Children’s Services (ACS) utilizing the Family Team Conference Model. The facilitator will ensure the safety and well-being of each child is assessed through a strength-based lens. Utilizing strong engagement skills, the facilitator will yield positive safety and permanency outcomes for children and families with the input from a multidisciplinary team and various stakeholders.
Responsibilities:
- Facilitate Family Team Conferences and other programmatic conferences;
- Provide direct supervision and support to Level I Facilitators
- Monitor the quality of conferences and documentation completed by Level I Facilitators and/or schedulers
- Hold weekly documented strength-based supervision with Level I or scheduler to promote growth and development
- Maintain fidelity to the Family Team Conference Model;
- Hold pre-conferences and debriefings
- Represent JCCA at Elevated Risk Conferences, Child Safety Conferences and other conferences as required
- Track activities related to FTC conferences including timeliness and completion rate
- Utilize data to assess strengths and areas for improvement
- Assess the strengths and needs of each child and family member in consultation with the interdisciplinary team and specialty consultants
- Ensure all recommendations to JCAA case planners and other service providers are communicated and captured in the action plan, FTC summary and progress notes
- Identify, evaluate and address any and all safety and risk concerns during conferences
- Participate in supervisory meetings and present FTC data
- Complete timely documentation in Connections and related agency database
- Facilitate a minimum of two to three conferences per day
- Attends inter/intra agency meetings to share information, develop policies and procedures
- Conduct Quality Assurance activities related to Family Team Conferencing (i.e., ensure SPR Tab is completed, ensure FTC’s are held within compliance timeframe and escalate cases approaching non- compliance).
- Monitor outcomes related to identifying performance trends, best practices, and opportunities for improvement;
- Develop and implement Quality Assurance measures to ensure quality service delivery to clients
- Provide ongoing education and training to clients and staff on Family Team Conferences
- Ensure adherence with agency, ACS and OCFS policies and procedures
- Other related duties as assigned by supervisor.
Qualifications:
- Master’s in Social Work, Psychology, Sociology, Criminal Justice or any closely related field from an accredited college or university.
- At least 2 years of experience in Child Welfare or closely related field;
- At least 1 year of supervisory experience or experience leading others.
- Must be proficient in Microsoft Excel, Word and PPT.
- Exceptional mediation skills to effectively facilitate group conference situations.
- Strong clinical skills and strength-based perspective.
- Strong writing skills.
- Excellent organizational skills
- Ability to handle multiple tasks at one time.
- Have a focus on outcomes and results.
- Knowledge of core principles and standards for NYC Child Welfare system.
- Ability to travel in and around New York City region and work remotely.
- Must be able to work remotely.
- Willing to work a flexible schedule that includes 2-3 evenings and at least one Saturday a month.
Preferred Experience:
- Preferred candidate will have at least two years of experience working in the child welfare system, FTC trained or familiar with group conferencing process. Excellent interpersonal skills, ability to work in a team environment and lead diverse groups toward resolution of differences.
Salary Range: $61,000.00 - $66,300.00 per year
